Ambulanter Entzug bei Opioidabhängigkeit und anschließende Abstinenztherapie

Zusammenfassung

Anliegen: Darstellung einer ambulanten Entgiftung von opioidabhängigen Patienten mit anschließender Abstinenztherapie.

Methode: Nach einer Umstellung auf Buprenorphin wurden die Patienten schrittweise abdosiert. Wenn möglich, wurde eine Abstinenztherapie mit Naltrexon angeschlossen.

Ergebnisse: Bei 6 von 10 Patienten mit Abhängigkeiten von Tramadol, Tilidin oder Oxycodon konnte die Entgiftungstherapie erfolgreich abgeschlossen werden. Bei 6 Patienten wurde eine Naltrexontherapie begonnen, die von 3 Patienten in den ersten 4 Wochen abgebrochen, bei den anderen 3 bis zu 1 Jahr fortgeführt wurde.

Schlussfolgerung: Mit dem genannten Vorgehen können Patienten mit Opioidabhängigkeit ambulant entgiftet und kann die weitere Abstinenz unterstützt werden.

Abstract

Objective: Report of an outpatient detoxification of opioid-analgetic dependent patients with a follow-up abstinence-therapy.

Methods: Patients were switched to Buprenorphin as a substitute and later the dosage was stepwise reduced to zero. Where possible, we continued with an abstinence-therapy using the opiate-antagonist Naltrexon.

Results: The consumed substances were Tramadol, Tilidin and Oxycodon. We carried out the detoxification-therapy with 10 patients over a period of 10–230 days. 6 patients successfully completed the treatment. Afterwards we started a therapy with 6 patients using the opiate-antagonist Naltrexon. 3 patients stopped the therapy after 1–4 weeks, the other 3 continued for 20–59 weeks.

Conclusions: The described procedure proved to be a practicable way to detoxicate patients with dependency on opioid-analgetics in an outpatient-setting and to support them during the following abstinence.
