Aims:

Duodenal diverticulum is common (6 – 20%). These may be a difficulty in cannulation of the common bile duct and are more frequent associated with biliary stones. The aims of the stady are calculate the prevalence of juxta-papillary diverticulum in patients with biliary stones and evaluate the influence of juxtapillary diverticulum on the success rate of the cannulation of the CBD in biliary stones pathology.

Methods:

A retrospective study that lead from April 2002 to November 2016. Were included 846 patients who benefited of a ERCP for CBD stones. The presence or absence of a juxta-papillary diverticulum was noted. We compared the success rate of cannulation of the common bile duct in patients with a juxta-papillary diverticulum (Group I) versus patients without diverticular papillae (group II).

Results:

The mean age of the patients was 58 ± 13 years with 515 women and 331 men (sex ratio M/F: 0.64). Patients with diverticular papillae (group I) accounted for 9.6% of the patients enrolled (81 patients). Group II had 765 patients (90.4%). The success rate of the cannulation of CBD was 96.4% in group II versus 86.5% in group I (p: 0.007). The overall rate of early complications was 6% in group II versus 7.4% in group I (p = 0.12).

Conclusions:

The presence of a juxta-papillary diverticulum appears to significantly decrease the success rate of CBD cannulation in biliary stones pathology without increasing the risk of early complications. However, prospective studies with large series are still necessary to confirm these results.
