Zusammenfassung

Obere gastrointestinale Blutungen sind ein häufiges internistisches Krankheitsbild. Dieser Fallbericht beschreibt eine schwere transfusionspflichtige obere GI-Blutung aufgrund eines Angiosarkoms. Wie in diesem Fall geschildert, kann die tatsächliche Ursache der GI-Blutung zunächst unklar bleiben. Hier ist eine wiederholte Histologie-Gewinnung zwingend notwendig, um die Diagnose zu sichern und so eine adäquate Therapie zu ermöglichen. Beim primären gastrointestinalen Angiosarkom, einem extrem seltenen, hochaggressiven malignen Tumor mit multifokalem Wachstum und frühzeitiger Metastasierung, stellt die radikale OP die einzige kurative Behandlungsoption dar. Eine frühzeitige Diagnosestellung ist daher entscheidend. Palliative Therapieansätze bei metastasiertem Angiosarkom sind aktuell Inhalt klinischer Studien.

Abstract

Upper gastrointestinal hemorrhage represents a common symptom in the internal medical practice. This Case Report depicts a severe gastrointestinal bleeding from an angiosarcoma that required multiple blood transfusion. As shown in this case, the actual cause may initially remain unclear. Repeated tissue sampling is mandatory to confirm a diagnosis and provide adequate treatment. Primary gastrointestinal angiosarcomas are extremely rare and highly aggressive malignant tumors that tend to grow multifocally and metastasise early. The only curative treatment option is radical surgery, thus an early stage diagnosis is crucial. Palliative care for metastatic angiosarkoma is currently a topic for clinical studies.
