Application of modified ALDRETE score after sedation and analgesia for gastrointestinal endoscopy results in shorter recovery time: a randomized trial

Aims The modified ALDRETE score evaluates the recovery of patients after administration of anesthesia for surgical procedures taking into account 5 parameters (airway, breathing, level of consciousness, heart rate, blood pressure). The purpose of this ongoing study is to evaluate the application of the modified ALDRETE score in monitoring of patients when they leave the recovery room of the endoscopy unit.

Methods Prospective randomized study enrolling patients undergoing diagnostic or therapeutic gastrointestinal endoscopy, with 1:1 randomization into two groups (Group A: assessment of patients in resuscitation room using the modified ALDRETE score and exit when ALDRETE score=9 and Group B: exit from resuscitation room after empirical evaluation). Possible late events or complications were recorded by contacting participants by telephone 24 hours after the endoscopy.

Results 1300 patients were randomized (Group A: 643 and Group B: 657). Of these, 709 (54.5%) are women with an average age of 62.5±10.3 years. The majority of participants underwent a colonoscopy (n=712, 54.8%), followed by a gastroscopy (n=298, 22.9%), while 671 (51.6%) subjects underwent an endoscopic procedure for the first time. Mean dose of medications used (midazolam, fentanyl and flumazenil) were comparable between the two groups. The average time spent in the resuscitation room was 18.9±5.9 minutes for all participants; however, this time was statistically significantly longer (p<0.001) for patients in group B (21.7±6.1 minutes) compared to group A (15.9±3.4 minutes). At 24-hours follow-up, significantly more patients in group B compared to group A reported headache after the examination (21.0% vs. 10.8%, p<0.001), while no differences were noted regarding abdominal pain, dizziness or nausea. Finally, all participants (99.7%) felt very or very safe leaving the recovery room, while 98.4% declared very or very happy with the examination process.

Conclusions Use of modified ALDRETE score to monitor patients after gastrointestinal endoscopy is associated with a shorter length of stay in the recovery room.
