EMR TORe: Treatment of excessively wide Gastro-Jejunal Anastomosis (GJA)

Abstract Text Excessively wide gastro-jejunal anastomosis (GJA) after gastric bypass can lead to weight gain or refractory dumping syndrome. An alternative endoscopic procedure, EMR-TORe, is proposed, refining the Transoral Outlet Reduction (TORe) method. A 36-year-old woman who regained significant weight post-surgery and a 68-year-old man suffering from severe dumping syndrome underwent this procedure. EMR-TORe involved circular mucosectomy at the anastomosis site, enhancing both patients' conditions. The first patient lost 8 kg, while the second reported an improvement in quality of life, no longer experiencing dumping syndrome. Although the technique aims to minimize stenosis, complete EMR can induce fibrosis, leading to effective clinical outcomes in narrowing the GJA [1].
