Novel approach to the management of persistent gastrogastric fistula post Roux-en-Y gastric bypass: Endoscopic Sleeve Gastroplasty of the remnant stomach

Abstract Text Gastrogastric fistulae are a rare but important complication of patients undergoing Roux-en-Y gastric bypass. They typically form in areas of chronic ischemia or inflammation, making them notoriously resistant to standard endoscopic closure techniques. We present a case of 62 y.o. female with abdominal pain and weight regain 25 years post gastric bypass. Endoscopy revealed a 2cm gastrogastric fistula that was recalcitrant to closure attempts using APC and endoscopic suturing of the fistula. The patient refused surgical revision, so we proceeded to endoscopic sleeve gastroplasty of the gastric remnant via the fistula. The patient was discharged home the same day and 3 months later had lost 14% of their total body weight. This case highlights a novel alternative approach for the management of symptomatic gastrogastric fistulae.
