Endoscopic Mucosal Resection with Snare Tip Soft Coagulation for a Rare Duodenal Tubulovillous Adenoma

Abstract Text RC, a 55 year old male, came in due to hematemesis. An EGD was done, and noted a 8mm sessile polypoid lesion at the second part of the duodenum with whitish discoloration of its overlying mucosa. On narrow band imaging, it has a regular caliber, regularly distributed vessel pattern and regular branched surface pattern. Injection of Voluven with methylene blue was done until a desired lift was achieved, and was removed via hot snare polypectomy. Snare tip soft coagulation was performed to significantly lessen the rate of adenoma recurrence. Two resolution clips were then deployed at the post polypectomy defect. The duodenal polyp specimen was diagnosed as Tubulovillous Adenoma, and its cauterized margins were negative for dysplasia [1] [2].
