A case of multiple colonic lipomatosis: An extraordinary endoscopic image

Introduction Gastrointestinal lipomatosis is characterized by the presence of a submucosal lobulated collection of mature fatty tissue. Colonic lipomatous lesions may be solitary or multiple, encapsulated lipomas or diffuse, discrete, unencapsulated lobules of adipose tissue [1] [2] [3] [4].

Case report We report the case of an 62-year-old female patient with a two month history of anemia. The patient denied melena or hematochezia and physical examination was unremarkable, with negative digital rectal exam for blood. Laboratory investigations showed significant microcytic anemia. The patient underwent endoscopic investigation of upper and lower gastrointestinal tract. Gastroscopy revealed duodenitis (Figure) and colonoscopy demonstrated the presence of multiple submucosal polypoid lesions of different sizes throughout the colon (Figure), especially in the descending and sigmoid colon. Bite-on-bite biopsies showed findings suggestive of lipomas. Small bowel wireless capsule excluded similar findings (Figure).

Conclusions Multiple colonic lipomatosis, like other lipomatous lesions of the colon, is a rare entity. Its benign nature makes it an “innocent” condition and it needs treatment only in the presence of complications.
