Gastric Polyps as a rare presentation of Metastatic Renal Cell Carcinoma: A case report

Metastatic renal cell carcinoma(RCC) is known for its ability to metastasize to unusual sites, but gastric involvement is exceedingly infrequent, occurring in 0.2% of all RCC cases [1]. We report here a case of metastatic RCC presenting as gastric polyps. By highlighting the significance of this rare occurrence, we aim to enhance clinicians’ awareness and improve patient outcomes.

68 year-old-female presented with non-specific gastrointestinal symptoms and a neck swelling. She had a history of radical nephrectomy for left Renal Cell Carcinoma (G2, T3a, N0). Subsequent CT scan demonstrated bilateral pulmonary nodules, pancreatic lesions, a gastric hyperdensity and an enlarged thyroid gland with multiple enhancing nodules. Differentials included metastasis or multi-focal neuroendocrine tumour. An urgent gastroscopy was performed that showed a few small gastric polyps, which were biopsied. Following this, she had an endoscopic ultrasound and biopsy samples were taken from pancreatic lesions. Histopathological analysis confirmed these lesions as metastases from previously treated RCC, with positive staining for PAX-8 and CD10 markers, supporting renal origin. The case was discussed in urology MDT for further oncological management.

This case highlights the diagnostic difficulties and the role of comprehensive histological evaluation in identifying metastatic disease. Recognition of this possibility can lead to timely diagnosis and appropriate management, impacting patient outcomes. Further research is needed to establish guidelines for surveillance and management of atypical metastatic sites in RCC.
