Pancreatic tuberculosis in a patient with Acquired Immunodeficiency Syndrome diagnosed by EUS-FNB

Abstract Text We present a case of a 32-year-old man with referred no documented history of HIV, presenting with unresponsive fever and abdominal pain. CT and MRI revealed a hypodense, multiloculated lesion in the uncinate process of the pancreas, with associated lymphadenopathies; PET-CT showed high metabolic activity in the same lesions. Laboratory tests confirmed HIV-positive status; EUS-FNB was crucial for the differential diagnosis, providing histochemical and PCR analysis that confirmed Mycobacterium tuberculosis complex infection. Pancreatic tuberculosis is an extremely rare and challenging diagnosis, often presenting with masses, cystic lesions or abscesses that frequently mimic pancreatic cancer; this case highlights the importance of EUS in the diagnostic work-up of a difficult-to-define lesion of the pancreas.
