Antegrade pancreatography vía EUS-guided pancreaticogastrostomy: lithotripsy and ‘ring drainage’ (gastro-pancreato-duodenal)
Abstract Text A 58-year-old man with abdominal pain related to a ductal stenosis, lithiasis and chronic pancreatitis. After a failed ERCP, an EUS-guided pancreatic duct intervention was considered. 1er procedure: EUS-guided pancreaticogastrostomy (PG) was achieved using a FCSEMS and co-axial plastic-stent as antimigration measure. 2º(10-weeks): Antegrade per-oral-pancreatoscopy through the PG. EH-lithotripsy allowed lithiasis fragmentation; and antegrade guidewire advancement until reaching the duodenum. FCSEMS deployment with a longer plastic-stent ensuring a ‘ring drainage’.3er (8-weeks): exchange of SEMS to two plastic-stents. Even though the limited evidence, a ring drainage seems to be more physiological, offering a double drainage, minimizing recurrence risk and pancreatic atrophy [1] [2] [3].
