Treatment of obstructive chronic pancreatitis with EUS-guided pancreatic approach
Abstract Text Endoscopic treatment of obstructive chronic pancreatitis is still a challenging scenario. EUS-guided pancreatic drainage is emerging as an alternative to ERCP for difficult cases. Here we report a case of a patient affected by obstructive chronic pancreatitis for a stone of the main pancreatic duct with recurrent episodes of acute pancreatitis and failure of pancreatic drainage during ERCP. EUS-guided pancreatic drainage was performed with a transgastric approach placing a pancreaticogastrostomy straight plastic stent and a transpapillary stent after a EUS-ERCP rendezvous. After four months, by an antegrade approach via the pancreaticogastrostomy an 8.5 Fr x 15 cm straight plastic stent with transgastric and transpapillary ends was positioned and a second transpapillary stent was placed retrogradely. EUS-guided pancreatic drainage allowed clinical resolution of symptoms and spontaneous clearance of the pancreatic stone.
