Electrohydraulic lithotripsy in obstructing pancreatic duct stones: a case report

Abstract Text Pancreatic duct stones (PDS) often occur in patients with chronic pancreatitis (CP). Endoscopic retrograde cholangiopancreatography (ERCP) has become the preferred approach for stones smaller than 5 mm in diameter. A 56-year-old female with acute on chronic pancreatitis with obstructing PDS at magnetic resonance (MR). The first attempt with traditional ERCP approach failed; a second ERCP with SpyGlass pancreatoscopy and electrohydraulic lithoptripsy was successfully carried out. A complete stone-crush was achieved by using EHL and fragments were removed. A pancreatic single pigtail stent 5 cm in length 5 fr in diameter was placed at the end. The patient was discharged after 48 hours and there was no clinical nor imaging evidence of recurrence during a 6-months follow-up.
