Proximal Intestinal Mucosal Ablation (PIMA): A novel procedure using the new Radiofrequency Vapor Ablation basket catheter to treat type 2 diabetes

Abstract Text Duodenal mucosal ablation is an endoscopic treatment for type 2 diabetes (T2D), but its potential has been limited by challenges in safely targeting the distal duodenum and proximal jejunum, which are regions excluded in gastric bypass surgery. Therefore, we have developed a novel procedure called Proximal Intestinal Mucosal Ablation (PIMA). PIMA uses the through-the-scope Radiofrequency Vapor Ablation basket catheter to deliver 250J of vapor ablation to the post-ampullary small intestines. We present a 56-year-old female with inadequately controlled T2D despite metformin who underwent PIMA with a colonoscope (Olympus HQ190L) under general anaesthesia. Procedure time was 73 minutes, ablation number was 35, and ablation length was 45 cm. They were discharged the next day with no serious adverse events or significant pain. At one month, their HbA1c fell by 20 mmol/mol.
