Observance of the European Society of Gastrointestinal Endoscopy 2019 and 2021 guidelines by the colonoscopy preparation instructions in Spain

Aims Our primary aim was assessing the observance of the European Society of Gastrointestinal Endoscopy (ESGE) guidelines regarding bowel preparation and management of antiplatelet and anticoagulant therapy in Spain. Secondary aims included describing all recommendations regarding issues not included in these guidelines and the implementation of new technologies [1] [2].

Methods We conducted a multicenter descriptive analysis assessing colonoscopy preparation instructions of Spanish public centres for adult out-patients. Private centers, incomplete instruction sets and in-patient/pediatric instructions were excluded. Instructions were retrieved between may 2022 and june 2023. Outcomes were categorized in the following groups: bowel cleansing administration, adjuvant cleansing medication, diet recommendations, management of current treatments, post procedure care, and recommendations for special populations.

Results A total of 84 colonoscopy preparation instructions were retrieved; seventy-nine from 41 Spanish centres were included in the final dataset.

Bowel cleansing administration: All laxatives employed were supported by the ESGE guidelines. Split-dose was counselled in 67 (84.8%) instructions.

Adjuvant cleansing recommendations such as other laxatives, prokinetics or enemas were included in 16 (19.3%) cases, with no center supporting the addition of simethicone.

Diet recommendations: Low-fibre diet was prescribed in 78 (98,7%) instruction sets, but 49 (62%) suggested a longer interval than the single day ESGE recommendation. Furthermore, 60 (75.9%) instructions recommended a clear liquid diet against the ESGE guidelines.

Current treatments: Over 90% of instructions did not include specific recommendations regarding anticoagulant or antiplatelet treatments, mostly submitting patients to their primary care providers.

Post procedure care: Driving was specifically forbidden after the procedure in 63 (79,7%) cases, without stating a specific interval. Patients were instructed to bring a companion in almost 90%.

Barely 11 (13,9%) preparation instructions used new technologies against ESGE recommendation to enhace information.

Conclusions The observance of the ESGE recommendations presents relevant issues with ample room for improvement. Errors concentrated in the diet section, while the lack of clear recommendations regarding antiplatelet and anticoagulant therapies contribute to the work overload in primary care
