PILONIDAL FISTULA: SURGERY WITH PRIMARY CLOSURE – MODIFIED BASCOM TECHNIQUE – TECHNICAL DETAILS AND TIPS

Introduction Surgical techniques for the treatment of pilonidal disease are described, including open excision with secondary healing or excision with primary closure with or without the use of a flap. The appropriate technique would be the one with the shortest healing time and lowest recurrence rate. The modified Bascom technique involves reducing the depth of the gluteal cleft and performing primary closure outside the midline using a flap advancement.
Objective The video demonstrates patients who underwent surgery using the modified Bascom technique for recurrent cases, highlighting technical details and tips for executing the procedures.
Method The video presents technical details of patients treated with the modified Bascom technique, who had previously undergone surgery with secondary healing without success. It demonstrates details of flap marking, tips for creating larger flaps, excision of the gluteal cleft, and primary closure, ensuring the effectiveness of the appropriate flap and wound drainage. No intraoperative complications occurred. A 1.5 cm opening in the epidermis and dermis was observed in one of the cases. No recurrence was noted during two years of follow-up.
Conclusion The modified Bascom technique with flap advancement and primary closure, executed according to technical principles, was effective for the treatment of recurrent pilonidal disease, with patients showing a healing time of up to six weeks.
