Zusammenfassung.

Die Strahlentherapie ist fester Bestandteil des multimodalen Therapiekonzeptes bei Patienten mit fortgeschrittenen Stadien eines Rektumkarzinoms. In der Kombination mit einer 5-FU(5-Fluorouracil)-haltigen Chemotherapie konnten sowohl die Lokalrezidiv- als auch die Überlebensraten verbessert werden. Inwieweit die neoadjuvante Radiochemotherapie bezüglich Resektabilität, Sphinktererhaltung und Toxizität zu besseren Ergebnissen führt als die postoperative Behandlung, wird derzeit in randomisierten Studien geprüft. In dieser Übersichtsarbeit wird anhand publizierter Studien der Stellenwert der Radio- und Radiochemotherapie bei resektablen Karzinomen sowie in besonderen Situationen wie bei primär nicht resektablen Karzinomen, bei Rektumkarzinomrezidiven und bei konservativem chirurgischen Vorgehen bei sehr tief sitzenden Karzinomen erläutert.

Radiation Therapy and Radiochemotherapy in Rectal Carcinoma.

Radiation therapy plays a major role in the multimodality treatment of patients with rectal cancer. In combination with a 5-FU (5-Fluorouracil) chemotherapy both local recurrence and survival rates were improved. It is the aim of current randomized studies to find out whether neoadjuvant radiochemotherapy improves the therapeutic results concerning tumor resectability, sphincter preservation and toxicity compared to postoperative therapy. Based on published studies in the following review the role of radiochemotherapy in resectable cancer and in special situation like in primarily not resectable cancer, in tumor recurrences and following conservative surgery is discussed.
