Behandlung schwieriger Skaphoidpseudarthrosen durch Knochentransplantation und zusätzliche Implantation der zweiten dorsalen Metakarpalarterie in der Technik nach Fernandez und Eggli - Erfahrungsbericht[*]

Zusammenfassung

Eine mögliche Behandlung der Skaphoidpseudarthrose mit durchblutungsgemindertem proximalen Fragment stellt die Technik nach Fernandez und Eggli (1995[4]) dar. Hierbei wird das Skaphoid nach Resektion der Pseudarthrose mit einem kortikospongiösen Beckenkammspan verblockt und zusätzlich die zweite dorsale Metakarpalarterie in das proximale Skaphoidfragment implantiert. In unserer Klinik wurden von Mai 1998 bis Juni 2000 sechs Patienten nach diesem Verfahren operiert. Es handelte sich dabei um fünf Patienten, bei denen, kernspintomografisch gesichert, ein avitales proximales Fragment bestand, und um einen Patienten, der bereits ohne Erfolg wegen einer Skaphoidpseudarthrose operiert worden war. Dabei kam es in fünf Fällen zu einer computertomografisch gesicherten knöchernen Konsolidierung der Pseudarthrose. In einem Fall kam es zur Vergrößerung des zystischen Bereichs mit Fortbestehen der Pseudarthrose, wobei jedoch keinerlei Beschwerden bestanden.

Das genannte Verfahren stellt unseres Erachtens eine geeignete Methode dar, bei problematischen Skaphoidpseudarthrosen eine bessere Knochenheilung zu ermöglichen. Insbesondere die konstante Lage der zweiten dorsalen Metakarpalarterie und die einfache Präparation sprechen für diese Operationstechnik.

Summary

One of the procedures for the treatment of scaphoid-nonunion with loss of blood supply to the proximal fragment involves the technique described by Fernandez and Eggli (1995[4]). After resection of the necrotic bone and inserting a corticocancellous bone graft from the iliac crest, the second dorsal intermetacarpal artery is implanted into the proximal fragment of the scaphoid. In our hospital, six patients underwent this procedure from May 1998 to June 2000 because of necrosis of the proximal pole or an unsuccessful previous operation. Union was achieved in five patients. In one case, nonunion persisted and the cystic area increased.

The described technique is suitable for improvement of bone healing of the scaphoid in complicated cases. The advantages of this technique lie in the predictable anatomy of the second intermetacarpal artery and the straightforward dissection.
