Zusammenfassung
Ein 62-jähriger Patient wurde wegen einer akuten nekrotisierenden Pankreatitis konservativ
antibiotisch behandelt. Im weiteren Verlauf entwickelten sich eine Gallengangstenose,
die durch eine mittels ERCP eingelegte biliodigestive Endoprothese versorgt wurde,
und ein Magenwandabszess, der durch eine endoskopisch-interventionelle Gastrostomie
mit Spülung und Drainage saniert werden konnte. Beide Komplikationen führten zu einem
protrahierten Verlauf, konnten jedoch letztlich erfolgreich behandelt werden. Es wird
auf die Bedeutung der leitliniengemäßen Therapie der akuten nekrotisierenden Pankreatitis
hingewiesen.
Abstract
A 62-year-old man with acute necrotizing pancreatitis was treated only with antibiotics
and supportive care. Subsequently he developed a common bile duct obstruction, which
could be treated by endoscopic biliary drainage with a plastic stent. Later on he
also developed an gastric wall abscess. Therefore he underwent endoscopic gastrostomy
with following irrigation and drainage of the abscess cavity. Both complications lead
to prolongation of the disease but could be handled successfully. It is important
to consider the actual guidelines for the therapy of acute necrotizing pancreatitis
to avoid complications.
