Zusammenfassung

Morbide Adipositas geht mit einer hohen Rate an pathologischen Begleiterkrankungen wie arterielle Hypertonie, Diabetes mellitus und Dyslipidämien einher. Durch eine effektive Gewichtsreduktion lässt sich eine deutliche Verbesserung dieser Begleiterkrankungen erreichen. Wir berichten über ein Patientengut von 220 Patienten nach Gastric banding (153 Frauen, 67 Männer) mit einem Durchschnittsalter von 37,8 Jahren. 35,9 % der Patienten wiesen präoperativ einen erhöhten Cholesterinspiegel auf, 21,8 % hatten ein erhöhtes LDL-Cholesterin. Pathologisch erniedrigt war der HDL-Cholesterinspiegel bei 23,2 % der Patienten. 2 Jahre nach Gastric Banding betrug die Reduktion des Übergewichtes 37,1 %. Gleichzeitig zeigte sich bereits nach einem Jahr ein signifikanter Abfall des LDL-Cholesterins (p = 0,002) und ein signifikanter Anstieg des HDL-Cholesterins (p = 0,001). Für das Gesamtcholesterin und die Triglyzeride ließen sich keine signifikanten Veränderungen nachweisen. Der Glukosespiegel sank im gleichen Zeitraum signifikant von 7,98 auf 5,24 mmol/l.

Abstract

Severe obesity is associated with a high incidence of comorbid diseases like diabetes, hypertonus and lipid disorders. Weight loss has a positive effect on these comorbidities. We studied the effect of gastric banding operation in 220 patients (153 women, 67 men) for weight reduction and its effect on the lipid status. The mean excessive weight loss (EWL-rate) in these patients was 37.1 %. 35.9 % had preoperative increased cholesterol, 21.8 % increased LDL-cholesterol level. Pathologic decreased HDL-cholesterol was found in 23.2 %. After one year a significant decrease of LDL-cholesterol (p = 0.002) and increase of HDL-cholesterol (p = 0.001) could be demonstrated. However no significant changes were found for triglycerids (p = 0.308) and total cholesterol (p = 0.359).
