Reaktivierte Epstein-Barr-Virusinfektion mit okulärer Myasthenie und Guillain-Barré-Syndrom

Summary

An acute polyradiculoneuropathy and ocular myasthenia were the neurological manifestations of (re)active Epstein-Barr Virus infection in a 56-year old previously healthy man. High level of early antigen antibodies in the serum, elevated CSF protein and CSF cells correlated with the clinical improvement unitil sixteen months after onset of the disease. This case is unique among reports of EBV-associated neurological diseases and reflects that the use of new diagnostic tools such as EBV antibodies and AChR antibodies may be an important supportive measure towards an accurate neurological diagnosis.

Zusammenfassung

Vorgestellt wird ein 56 Jahre alter Patient, bei dem es zur Kombination eines akuten Guillain-Barré-Syndroms mit ZNS-Beteiligung und einer okulären Myasthenie mit Thymom gekommen war. Ursächlich konnte durch den typischen Titerverlauf der Nachweis einer reaktivierten Epstein-Barr-Virusinfektion erbracht werden. Somit handelte es sich hier um eine EBV-assoziierte neurologische Erkrankung in einer Kombination, wie sie unseres Wissens bisher noch nicht beschrieben worden ist. Die möglichen pathogenetischen Mechanismen werden diskutiert.
