Die intrakranielle Kollateralversorgung bei Stenosen und Verschlüssen der Arteria carotis interna

Summary
We prospectively investigated intracranial collateralisation by transcranial Doppler sonography in 58 cases of stenosis (>50%) or occlusion of the internal carotid artery (ICA). Intracranial collateralisation was observed in severe ICA stenonis (>90%) and occlusion. The most frequent intracerebral collateral was the contralateral anterior cerebral artery via the anterior communicating artery (90% in cases of ICA-occlusions). A collateral supply via the ophthalmic artery occurred in 53% and/or the vertebrobasilar system in 47% of the patients with ICA occlusion.
In patients with TIA or stroke, intracranial collateralisation via the opthalmic artery was observed in 61%, in contrast to 28% of patients without neurological symptoms.
Zusammenfassung
Bei 58 Patienten mit Stenosen (>50%) oder Verschlüssen der Arteria carotis interna (ACI) untersuchten wir mit der transkraniellen Dopplersonographie die intrakranielle Kollateralversorgung. Eine Kollateralisierung setzte ab einer einseitigen Stenosierung der ACI > 90% ein. Die häufigste Kollateralversorgung erfolgte dabei über die kontralaterale Arteria cerebri anterior (ACA) via Arteria communicans anterior (bei ACI-Verschlüssen in 90%). Eine Kollateralversorgung über die Arteria ophthalmica bzw. das vertebrobasiläre System erfolgte bei ACI-Verschlüssen in 53% bzw. 47%.
Während der Anteil an Kollateralisierungen über die kontralaterale Arteria cerebri anterior bzw. die Vertebralarterien bei symptomatischen bzw. asymptomatischen Patienten mit ACI-Verschluß gleich war, überwog der Anteil der Ophthalmikakollateralen bei den symptomatischen (61%) gegenüber den asymptomatischen Patienten (28%).
