Rezidivierende Carbamazepin-Intoxikation durch eine Angehörige

Summary

We report on a male patient who presented himself several times with fluctuating symptoms which could predominantly be located in the brainstem and the cerebellum.

On account of a CSF pleocytosis and protein elevation without disturbance of the blood-CSF barrier, a CNS inflammation initially was suspected. Finally the symptoms could be attributed to a non-accidental chronic recurrent carb-amazepine intoxication. The drug had been given to the patient by his mentally ill wife without his knowledge.

Zusammenfassung

Es wird über einen Patienten berichtet, der mit einer überwiegend dem Hirnstamm und Zerebellum zuzuordnenden fluktuierenden Symptomatik mehrfach in stationärer neurologischer Behandlung war. Aufgrund einer in mehreren Lumbalpunktionen nachweisbaren Pleozytose und Eiweißvermehrung ohne Schrankenstörung wurde zunächst eine entzündliche Genese der Beschwerden vermutet.

Letztlich konnte jedoch eine chronisch-rezi-divierende Carbamazepin-Intoxikation als Ursache der Symptomatik nachgewiesen werden. Die Substanz war dem Patienten unwissentlich durch die psychisch kranke Ehefrau verabreicht worden.
