In der Vagina siedeln mindestens 250 Bakterienarten, die in einer variablen Balance mit den dominierenden Laktobazillen stehen. Nur ein Bruchteil dieser Mikroorganismen löst gynäkologisch-urologische Infektionen aus, die dann spezifisch behandelt werden müssen – je nach Erregerart auch unter Einbezug des Sexualpartners. Die meisten gynäkologischen Infektionen erhöhen in der Schwangerschaft zudem das Risiko für Frühgeburten.

  • Laktobazillen dominieren die vaginale Mikrobiota in einem balancierten Ökosystem.

  • Das Nativpräparat ist die wichtigste Maßnahme zur Unterscheidung von normaler und abnormaler Vaginalflora.

  • Bei BV erstarken L. iners und zahlreiche BV-assoziierte Bakterien meist mit G. vaginalis, und bilden ab einer kritischen Mindestmenge und Zusammensetzung polymikrobielle Biofilme. BV disponiert zu aszendierenden und sexuell übertragbaren Infektionen sowie zu Spätaborten, Frühgeburten und puerperalen Infektionen. Zur Therapie kommen Clindamycin oder Metronidazol oral oder vaginal oder Dequaliniumchlorid infrage. Oral oder vaginal verabreichte Laktobazillen und Mittel, die den pH-Wert normalisieren, reduzieren die Rezidivquote von BV und Dysbiosen. Die BV ist sexuell übertragbar, eine Partnertherapie aber bisher erfolglos, da der Biofilm nicht beseitigt wird.

  • Die VVK tritt bei Abwehrschwächen nach Kolonisation mit (meist) C. albicans bei prämenopausalen Frauen auf. Typisch, aber nicht verlässlich, ist vestibulärer Juckreiz. Zur Diagnose dient der Nachweis von Pseudohyphen, bei Zweifeln oder chronischen Rezidiven mit der Pilzkultur.

  • Trichomoniasis ist sexuell übertragbar, sie triggert Frühgeburten. Die Therapie erfolgt mit 2 g Metronidazol oral für alle Partner.

  • Die aerobe Vaginitis ist eine inflammatorische Immunerkrankung. Frühgeburten kommen dabei gehäuft vor. Sie wird bei Kolpitis und gelb-grünem Fluor durch einen hohen pH-Wert mit Nachweis vieler toxischer Leukozyten, Parabasalzellen und einer kokkoiden Flora ohne Clue Cells diagnostiziert.

  • Eine Salpingitis tritt als akut aszendierende Genitalinfektion meist bei jungen Frauen auf und wird polymikrobiell verursacht. Chlamydien sind in etwa 30% der Fälle beteiligt. Mycoplasma genitalium muss als wichtiger sexuell übertragbarer Erreger beachtet werden. Ein Erregernachweis ist nur laparoskopisch durch Abstrich vom Fimbrienende möglich. Bei der 10- bis 14-tägigen Therapie werden Antibiotikakombinationen empfohlen, die das mögliche Keimspektrum erfassen. Eine Partnertherapie muss ggf. erfolgen.
