Zusammenfassung

Die externe Strahlenbehandlung der endokrinen Orbitopathie ist eine strahlentherapeutische Routinebehandlung. Gleichwohl sind die Zuweisungen zu dieser Therapie zurückgegangen.

Wirkmechanismus der Strahlentherapie ist die Suppression der Lymphozyten und Fibroblasten, die durch eine Autoimmunreaktion aktiviert wurden und zu einer konsekutiven, raumfordernden Verdickung der Augenmuskeln führen.

Für die Behandlung einer Entzündungsreaktion im Frühstadium reichen Einzeldosen von 0,3 Gy und Gesamtdosen von 2,4 Gy aus, chronischere Fälle benötigen Einzeldosen von 2 Gy bis zu einer Gesamtdosis von 18 Gy.

Eine Befundbesserung wird in 80 % der Fälle erreicht. In bis zu 91 % der Fälle kann eine Kortikoidmedikation beendet oder reduziert werden.

Deshalb sollte die externe Strahlentherapie der endokrinen Orbitopathie zukünftig leitliniengerecht wieder häufiger Anwendung finden.

Abstract

External beam radiation therapy is a routine procedure in radiotherapy. Nevertheless referrals to therapy have been abated.

Radiotherapy acts by suppression of lymphocytes and fibroblasts that have been activated by an autoimmune reaction leading to thickening of orbital muscles.

In case of early inflammation 2,4 Gy total dose at single doses of 0,3 Gy are sufficient, while more chronic cases require single doses of 2 Gy up to a total dose of 18 Gy.

In 80 % of the cases improvement can be observed. In up to 91 % corticosteroid intake can be tapered off completely or remarkably reduced.

Therefore external beam radiation therapy should be used again more frequently according the actually valid therapy guidelines.
