Insertion of multiple plastic stents into the bile duct may result in migration of the initially placed stent during insertion of the subsequent stent. Migration of plastic stents is a particular problem in patients with altered anatomy as handling of the scope in such cases is more complicated, and attempts to remove the migrated stent may lead to ERCP failure. Therefore, the double-wire stent-antimigration technique presented by Kachi et al. is of important endoscopic value for several reasons: (a) it increases our armamentarium of techniques to prevent stent migration during deployment; (b) it shows a case of extreme ERCP in a patient with altered anatomy; and (c) the technique may also be applied during routine ERCP in patients with intact anatomy.
