Spritzenphobie bei Diabetes

ZUSAMMENFASSUNG

Diabetespatienten mit Spritzenphobien beschreiben im Vorfeld und während des Spritzvorgangs bzw. beim Kathetersetzen panikartige Angstzustände, häufig mit Ohnmacht. In der Folge vermeiden Betroffene das Insulinspritzen, wann immer möglich. Die klinische Relevanz der eher seltenen und vorwiegend bei Kindern vorkommenden Angststörung ergibt sich aus der stark erschwerten Umsetzung der Insulintherapie mit dem Ziel des Erreichens normnaher Glukosewerte. Bei der Therapie hat sich die Methode der graduierten Exposition als wirksam erwiesen. Bei Patienten mit Furcht vor Einstichschmerzen helfen anfänglich Entspannungsmethoden, Kontrolle über das Angstgeschehen zu erleben. Bei zurückliegenden Ohnmachtserfahrungen können Patienten diese durch die Methode der „angewandten Anspannung“ kontrollieren. Die Reduktion phobischer Ängste gelingt mittelfristig jedoch nur durch Kontrollerfahrungen ohne Anwendung von Sicherheits- und Vermeidungsverhalten.
