Zusammenfassung

Gonarthrose gehört zu den häufigsten Krankheitsbildern in Deutschland und verursacht Invalidität, Arbeitsunfähigkeit und Einbußen in der Lebensqualität. Dies führt zu Herausforderungen für konservative Therapien, insbesondere die Manuelle Therapie, die trotz wissenschaftlicher Empfehlungen insuffizient verordnet wird. Ziel der Arbeit war, hemmende und fördernde Faktoren im Praxisalltag von Ärzten, Physiotherapeuten und Patienten bei der Wahl von Manueller Therapie bei Gonarthrose herauszufinden. In der qualitativen Studie wurden 23 Ärzte, Physiotherapeuten und Patienten in halbstrukturierten Interviews befragt. Die Transkriptionen wurden mittels zusammenfassender Inhaltsanalyse ausgewertet. Daraus wurden induktiv Kategorien gebildet.

Vier Kategorien wurden identifiziert, die hemmende und fördernde Aspekte bei der Wahl von Manueller Therapie bei Gonarthrose lieferten: „Bedingungen im Gesundheitssystem“, „Erfahrungen mit der Therapiemethode Manuelle Therapie“, „Die Rolle der Patienten“ und „Zusammenarbeit“. Diese geben Aufschlüsse bezüglich der Entscheidungen der Gesundheitsdienstleister und Patienten bei der Wahl von Manueller Therapie in der konservativen Therapie von Gonarthrose.

Abstract

Osteoarthrosis of the knee is one of the most common diseases in Germany and causes invalidity, inability to work and loss of quality of life. This leads to challenges for conservative management especially manual therapy, which is prescribed insufficiently in spite of scientific recommendation. The aim of this study was to find barriers and facilitators for the choice of manual therapy for osteoarthrosis of the knee in physicians’, physiotherapists’ and patients’ daily practice. In this qualitative study semi-structured interviews were performed with 23 physicians, physiotherapists and patients. The transcriptions were evaluated using summarizing content analysis and categories were developed inductively.

Four categories providing inhibiting and facilitating aspects for choosing manual therapy for osteoarthrosis of the knee were identified: „structure and conditions of the health system“, „experience with the therapy method manual therapy“, „the role of the patients“ and „collaboration“. These categories provide information concerning the decisions of health service providers and patients for choosing manual therapy in the conservative management of osteoarthrosis of the knee.
