Palliative Viszeralchirurgie

In der palliativen Behandlung steht die Linderung von Schmerzen, Atemnot, Übelkeit und anderen Krankheitsbeschwerden, die am Lebensende und bei unheilbaren Erkrankungen auftreten, im Vordergrund. Die Therapie dieser Beschwerden und Notfallsituationen ist immer individuell und abhängig von der jeweiligen Erkrankungssituation und dem Patientenwillen. Im vorliegenden Artikel befassen wir uns mit den Besonderheiten der Behandlung von palliativen Patienten im perioperativen Verlauf und bei den häufigsten Komplikationen.

  • Palliativpatienten stellen Behandler vor besondere Herausforderungen.

  • In der medizinischen Ausbildung steht die Heilung im Vordergrund, und die meisten onkologischen Therapien zielen daher auf eine Heilung oder maximale Lebensverlängerung ab. Bei einem palliativen Behandlungskonzept wird jedoch der Tod akzeptiert, und die Zeitspanne, bis er eintritt, spielt häufig nur noch eine untergeordnete Rolle.

  • Der Fokus der Behandlung liegt auf der Linderung von Schmerzen und anderen Krankheitssymptomen sowie dem Erhalt der Selbstständigkeit.

  • Eine Behandlung sollte möglichst so ausgelegt sein, dass Patienten zu Hause oder ambulant betreut werden können und nur so wenig Zeit wie möglich im Krankenhaus verbringen müssen.

  • Eine frühzeitige Einleitung einer palliativen Therapie kann die Lebensqualität am Lebensende deutlich verbessern.

  • Die palliative Therapie umfasst nicht nur die Anwendung einer intensivierten Schmerztherapie. Die Chirurgie spielt eine entscheidende Rolle in der Behandlung von Komplikationen, die im Rahmen von unheilbaren, meist onkologischen Erkrankungen auftreten, wie Stenosen, Perforationen oder Blutungen.

  • Das Spektrum der Behandlung reicht dabei von kleinen Interventionen bis hin zu ausgedehnten Resektionen je nach Situation und Zustand des Patienten.
