Procedure duration is an important limitation of endoscopic submucosal dissection
(ESD), although the benefits of R0 resection for patients are evident. In this randomized
trial of ESD using the pocket-creation method, clip traction resulted in faster dissection
speeds and reduced procedure times compared with no clip traction. Faster dissection
may help to align ESD more favorably with piecemeal endoscopic mucosal resection in
terms of procedure duration, while maintaining specimen quality and reducing the risk
of local recurrence.
