Spezielle Orthopädische Geriatrie (SOG) – G-BA Modellprojekt zur Versorgung orthogeriatrischer Patienten mit elektiven orthopädischen Eingriffen

Zusammenfassung

Durch die demographische Entwicklung ist in den nächsten Jahren und Jahrzehnten mit einer deutlichen Zunahme geriatrischer Patienten im deutschen Gesundheitssystem zu rechnen. So wird auch die Anzahl älterer Patienten in den Fachgebieten Unfallchirurgie und Orthopädie weiter erheblich zunehmen. Während sich die Unfallchirurgie diesbezüglich durch zertifizierte Alterstraumazentren, der Richtlinie zur Versorgung der hüftgelenknahen Femurfrakturen des Gemeinsamen Bundesausschuss (G-BA) und der geriatrischen frührehabilitativen Komplexbehandlung mittlerweile gut aufgestellt hat, existieren in der Orthopädie mit überwiegend elektiven Eingriffen bisher noch keine adäquaten Antworten hierauf. Doch gerade in der Primär- und Revisionsendoprothetik sowie auch bei Wirbelsäuleneingriffen ist mit dramatisch steigenden Zahlen älterer, multimorbider Patienten zu rechnen. Das G-BA Modellprojekt „Spezielle Orthopädische Geriatrie (SOG)“ evaluiert auf wissenschaftlicher Grundlage erstmals in Deutschland ein speziell auf die Elektivsituation abgestimmtes, innovatives Versorgungskonzept in der interdisziplinären Behandlung orthogeriatrischer Patienten bei primärem Hüft- und Kniegelenkersatz. In einer ersten Zwischenanalyse zeigten sich bereits vielversprechende Ergebnisse. Im Erfolgsfall könnte das Konzept für die Behandlung weiterer Erkrankungen übernommen und als neue Komplexbehandlung für elektive chirurgische Eingriffe etabliert werden.

Abstract

Due to demographic trends, a significant increase in geriatric patients in the German healthcare system is to be expected in the coming years and decades. The number of elderly patients in the specialist areas of trauma surgery and orthopaedics will continue to increase significantly. While trauma surgery is now well positioned in this respect thanks to certified geriatric trauma centers, the guideline on the treatment of femur fractures near the hip joint issued by the Federal Joint Committee (G-BA) and geriatric early rehabilitation complex treatment, there are still no adequate answers to this situation in orthopaedics, where the majority of procedures are elective. However, the number of older, multimorbid patients is expected to increase dramatically, particularly in primary and revision arthroplasty as well as in spinal surgery. The G-BA model project “Special Orthopaedic Geriatrics (SOG)” is the first scientific evaluation in Germany of an innovative care concept specifically designed for the elective situation in the interdisciplinary treatment of orthogeriatric patients undergoing primary hip and knee replacement. An initial interim analysis has already shown promising results. If successful, the concept could be adopted for the treatment of other diseases and established as a new complex treatment for elective surgical procedures.
