Zusammenfassung
Beschrieben wird der Fall eines 23-jährigen Mannes mit Swyer-James-McLeod-Syndrom (SJMS) und gut kontrolliertem Asthma, der an einer Expedition in den indischen Himalaya teilnehmen wollte. Trotz sorgfältiger Vorbereitung musste er die Expedition auf einer Höhe von 3000 m aufgrund eines fieberhaften Infekts abbrechen. Diskutiert werden höhenspezifische Risiken und Herausforderungen für Individuen mit pulmonalen Vorerkrankungen bei Höhenexposition. Die Asthmatherapie wurde „prophylaktisch“ eskaliert, um eine, durch externe Faktoren (Höhe, Kälte, körperliche Belastung), getriggerte Exazerbation zu verhindern. Eine defensive Steigtaktik (Schlafhöhendistanz 400–500 m) wurde geplant. Dieses Vorgehen basiert auf theoretischen Überlegungen und eigener Erfahrung. Leitlinien hierzu existieren nicht.
Abstract
We describe a case of a 23-year-old male with Swyer-James-McLeod syndrome (SJMS) and well-controlled asthma who planned to take part in an expedition to the Indian Himalayas. Despite careful physical and medical preparation, he had to abort the expedition at an altitude of 3,000 meters a. s. l. due to a feverish infection. Discussed are altitude-specific risks and challenges for individuals with pre-existing pulmonary conditions when exposed to altitude. Asthma therapy was escalated prophylactically to prevent an exacerbation triggered by external factors (altitude, cold, physical exertion). A defensive climbing tactic (sleeping altitude distance of 400–500 m) was planned. This approach is based on theoretical considerations and personal experience. No guidelines exist for this.
