The role of the functional luminal imaging probe (FLIP) in predicting clinical response to gastric peroral endoscopic myotomy (G-POEM) was investigated in a prospective trial. Clinical response was achieved in 64 of 90 patients (71.1%) at 6-month (median) follow-up. Mean distensibility index increased significantly in both responders and non-responders after G-POEM, but remained persistently elevated only among responders at 6 months. These promising results require additional studies to optimize and standardize FLIP protocols in G-POEM.
