Sektorenübergreifende Versorgung – Gamechanger für die Chirurgie?

Definition und Zielsetzung
Die medizinische Versorgung in Deutschland ist traditionell in unterschiedliche Versorgungssektoren gegliedert – insbesondere in die ambulante, stationäre sowie rehabilitative und pflegerische Versorgung. Diese sektorale Trennung hat historisch gewachsene und organisatorische Gründe, führt jedoch in der Praxis häufig zu Brüchen in der Versorgungskette, Informationsverlusten und ineffizienten Behandlungsverläufen. Besonders in der Chirurgie, wo komplexe Behandlungsabläufe mit hohem Koordinationsbedarf bestehen, wird die Notwendigkeit einer sektorenübergreifenden Versorgung deutlich [1].
Ziel sektorenübergreifender Versorgung ist es, die Fragmentierung im Gesundheitssystem zu überwinden und eine patientenzentrierte, durchgängig koordinierte Behandlung zu ermöglichen. Eine zentrale Herausforderung besteht darin, den Informationsfluss über die Sektorengrenzen hinweg sicherzustellen, um Kontinuität und Qualität der Versorgung zu gewährleisten [2].
