Expulsion of a SALIBI-Clamp from the Internal Carotid Artery in the Neck by Infection

Summary
A left infraclinoid carotid aneurysm (3 × 2·5 × 2 cm) in a 60-year-old woman presented with total ophthalmoplegia and trigeminal symptoms.
A staged internal carotid occlusion in the neck was performed using a SALIBI-clamp[*]. A chronic infectious fistula developed (Klebsiella aerogenes). When superficial debridement failed, operative removal of the SALIBI-clamp was planned seven months after its insertion. The clamp was found subcutaneously without any relation to the internal carotid artery and was easily removed. Subsequent catheter angiography proved the left internal carotid to be completely occluded in the neck. No cross filling of the aneurysm could be demonstrated from the right carotid nor the vertebro-basilar arteries. The neurological deficit remained unchanged. The wound closed promptly without further signs of infection.
Zusammenfassung
Ein linksseitiges, 3 × 2,5 × 2 cm großes, infraclinoidales Carotis-Aneurysma bei einer 60jährigen Frau manifestierte sich durch eine totale Ophthalmoplegie und leichte Trigeminusanfälle.
Mit einer SALIBI-Klemme[*] wurde eine gestufte Okklusion der linken Arteria carotis interna cervikal durchgeführt. Es bildete sich eine chronische Wundfistel (bakt. Klebsiella aerogenes) aus. Nach einer erfolglosen Wundrevision wurde die operative Entfernung der SALIBI-Klemme 7 Monate nach Einlage vorgesehen. Die Klemme fand sich subcutan ohne anatomische Beziehung zur Arteria carotis interna; sie wurde mühelos entfernt. Die nachfolgende Katheterangiographie zeigte einen kompletten Verschluß der cervikalen Carotis interna. Eine Kollateralversorgung des Aneurysma war weder von der rechten Arteria carotis noch vom Vertebralis-Basilaris-Kreislauf her nachweisbar. Die neurologischen Ausfälle blieben konstant. Die Wunde schloß sich per primam ohne weitere Infektionszeichen.
