Relapsing Form of Subacute Necrotizing Encephalomyelopathy

Abstract

Clinico-pathological report on a boy aged 7 years with negative family history who suffered from a chronically relapsing neurological disorder starting in the early post-natal period. Non-specific infections were followed by ataxia, motor weakness and visual disorders. There were prolonged periods of clinical improvement with re-development of previously lost static and motor skils. The terminal state was that of dementia, tetraparesis, optic atrophy and decerebration. Necropsy showed fatty transformation of the liver with nuclear vacuolation and subacute necrotizing encephalomyelopathy with lesions of various ages, predominantly affecting the corpora striata and dentate nuclei. There was unilateral focal involvement of substantia nigra and mammillary body, demyelination of the optic pathways, symmetric (primary) degeneration of the corticospinal tracts and slight peripheral neuropathy, the brainstem tegmentum being preserved. The clinical and morbid anatomical peculiarities of this observation demonstrate the variability in the natural course of this disorder. No biochemical studies of thiamine metabolism were done, and no specific therapeutic trials were performed.

Zusammenfassung

Klinisch-pathologischer Bericht über einen 7jährigen Knaben mit negativer Familienanamnese, der an einem in der frühen Postnatalperiode einsetzenden chronisch-remittierenden neurologischen Prozeß litt. Unspezifische, fieberhafte Infekte waren von Ataxie, Schwäche und Sehstörungen gefolgt. Anhaltende Remissionsphasen gingen mit Wiedererlangung verlorener statischer und motorischer Funktionen einher. Tod im Endstadium tiefer Demenz, Tetraparese, Blindheit mit Opticusatrophie und Decerebration. Autoptisch fanden sich Leberverfettung mit zahlreichen „Lochkernen” in Leberzellen und subakute nekrotisierende Encephalomyelopathie mit Läsionen verschiedenen Alters. Neben Vorzugsbefall des Striatum und der Zahnkerne fand sich einseitige fokale Schädigung der Nigra und des Corpus mamillare, Entmarkung der optischen Bahnen und primäre symmetrische Degeneration der Corticospinalbahnen sowie leichte periphere Neuropathie. Die Hirnstammhaube war verschont. Diese Beobachtung demonstriert die klinische und morphologische Vielgestaltigkeit der Erkrankung. Biochemische Untersuchungen des Thiaminstoffwechsels und gezielte Therapieversuche erfolgten nicht.
