Periphere Polyneuropathie und beidseitige Optikusneuropathie unter Behandlung einer chronischen Hepatitis C

Zusammenfassung

Anamnese und klinischer Befund: Eine 54-jährige Patientin mit kurz zuvor auswärts gesicherter chronischer Hepatitis C, Genotyp Ib und positiver HCV-RNA wurde zur Einleitung einer Interferontherapie in unsere Klinik stationär aufgenommen.

Untersuchungen: Der körperliche Untersuchungsbefund war unauffällig. Im Ultraschall des Abdomens stellte sich die Leber mit leicht verdichteter Struktur dar, ansonsten bestand kein pathologischer Befund. Das Labor zeigte eine gering erhöhte Glutamat-Pyruvat-Transaminase (GPT) mit 46 U/l bei einer HCV-RNA von 769 000 U/ml. Alle übrigen Laborwerte lagen im Normbereich. Es gab keinen Hinweis für Autoantikörper.

Therapie und Verlauf: Es erfolgte eine einwöchige Induktionstherapie mit Interferon-α (IFN-α2a) plus Ribavirin sowie Amantadin und danach die Einleitung der Erhaltungstherapie mit pegyliertem Interferon-α (PEG-IFN-α2a) plus Ribavirin sowie Amantadin. 2,5 Monaten später war eine komplette Viruselimination erreicht. 5 Monate nach Therapiebeginn entwickelte sich bei der Patientin eine schmerzhafte periphere Neuropathie der Extremitäten, und einen weiteren Monat später kam es zu schweren Sehstörungen bei Optikusneuropathie mit Stauungspapillen beiderseits. Gleichzeitig war die periphere Polyneuropathie progredient mit zusätzlichen motorischen Ausfällen. Die Interferontherapie wurde abgebrochen, und die Patientin erhielt hochdosiert Kortikosteroide, worunter sich die neuropathischen Beschwerden besserten. 8 Monate später war als Residualzustand eine beidseitige diskrete Visusminderung bei Optikusatrophie beidseits zu beobachten.

Folgerung: Im Allgemeinen sind die unter Therapie mit Interferon-α als auch mit pegyliertem Interferon-α auftretenden Nebenwirkungen gering und gut tolerierbar. Dennoch wurden, wie in unserem Fall, in Einzelfällen darunter schwere periphere Neuropathien sowie Optikusneuropathien beobachtet. Bei Auftreten dieser Veränderungen muss eine Interferontherapie sofort unterbrochen werden, da es sonst zu schweren Nervenschädigungen kommen kann.

Summary

History and clinical findings: A 54-year-old female patient with recently confirmed chronic hepatitis C, genotype Ib and positive HCV-RNA was admitted for interferon induction therapy.

Examinations: Physical examination findings were normal. Ultrasound examination of the abdomen revealed slightly compacted liver structure with otherwise no pathological findings. Laboratory results showed marginally increased GPT value at 46 U/l with HCV-RNA of 769 000 U/ml. All other laboratory results were within the reference range. There was no indication for autoantibodies.

Therapy and course: After one-week induction therapy with interferon-α (IFN-α2a) plus ribavirin and amantadine, maintenance therapy with pegylated interferon-α (PEG-IFN-α2a) plus ribavirin and amantadine was initiated. Complete virus elimination was achieved after two and a half months. Five months after onset of therapy, painful peripheral neuropathy of the limbs occured and after one more month, severe vision impairment with opticus neuropathy and bilateral papilledema developed. Simultaneously, peripheral polyneuropathy progressed with additional motoric deficits. Interferon therapy was terminated and the patient received high-dose corticosteroids which resulted in an improvement of neuropathic complaints. Eight months later, only discrete bilateral vision impairment with optic nerve atrophy remained.

Conclusion: Side effects occurring with interferon-α therapy and pegylated interferon-α therapy are generally low and well tolerated. However, in individual cases – as in our patient – side effects including severe peripheral neuropathy and optic neuropathy have been observed. In these cases, interferon therapy must be terminated immediately as severe damage of nerves may result.
