Zusammenfassung

Übelkeit und Erbrechen sind 2 verschiedene Symptome mit wechselseitiger Beeinflussung und erheblichem Einfluss auf die Lebensqualität. Ihre Prävalenz liegt je nach untersuchtem Kollektiv bei 30–60 % der Palliativpatienten. Diese Symptome sind jedoch nur die Endstrecke komplexer und vielfältiger Interaktionen von Großhirnrinde, Chemotriggerzone, Brechzentrum, Vestibularzentrum, Okulomotoriuskernen, Nucleus tractus solitarii sowie anderen autonomen Afferenzen. Anhand einer sorgfältigen Anamnese und gezielten Untersuchung lassen sich Hinweise auf die vorrangige Ätiologie gewinnen, wobei gerade in fortgeschrittenen Erkrankungssituationen Multikausalität die Regel ist. Große Bedeutung kommt kausalen Therapien zu, da viele Ursachen auch in der Palliativsituation reversibel sind. Die medikamentöse symptomatische Therapie sollte sich an pathophysiologischen Zusammenhängen orientieren, wobei im Rahmen eines pragmatischen Behandlungskonzeptes häufig Kombinationstherapien erforderlich sind. Hier gilt es, sich gegenseitig aufhebende Wirkungen zu vermeiden. Zum multimodalen Behandlungskonzept gehören eine gezielte Ernährungsberatung ebenso wie psychologische und physikalische Verfahren.

Abstract

Nausea and vomiting are separate but related symptoms affecting 30 up to 60 % of patients with advanced disease. They have major impact on their life quality. Complex interactions between higher centers in the brain, chemoreceptor trigger zone, vomiting center, vestibular organ and n. oculomotorius as well as autonomic afferents lead to nausea and vomiting. A careful history, focused physical examination and appropriate investigations are needed to elucidate the syndrome and its causes, so that therapy is rational. In far advanced stages, however, the etiology often appears to be multicausal and multidimensional. Thus, a more pragmatic approach with combined drug regimens and complementary dietary advice and psychotherapy might be more effective.
