Schwere corticoidrefraktäre Autoimmunthrombozytopenie bei Sharp-Syndrom

Zusammenfassung

Anamnese und klinischer Befund: Eine 18-jährige Patientin mit seit 2 Jahren bekanntem Sharp-Syndrom wurde wegen einer schweren Thrombozytopenie stationär aufgenommen. Hinsichtlich der Kollagenose-spezifischen Beschwerden bestand zum Aufnahmezeitpunkt lediglich ein Raynaud-Syndrom. Die Patientin war subjektiv kaum beeinträchtigt, Blutungssymptome lagen nicht vor, eine Splenomegalie war klinisch nicht nachweisbar.

Untersuchungen: Die bildgebende Diagnostik erbrachte keine richtungweisenden Befunde. Laborchemisch waren die Thrombozyten mit 5 Gpt/l (Normbereich 150 – 400 Gpt/l) supprimiert. Die Knochenmarkszytologie ergab eine sekundär immunologisch vermittelte hypermegakaryozytäre Thrombozytopenie.

Therapie und Verlauf: Initial wurde über 4 Wochen mit 100 mg/d Prednisolon therapiert, ohne dass ein signifikanter Effekt zu beobachten war. Eine Splenektomie lehnte die Patientin ab. Daraufhin erhielt sie 4 Infusionen Rituximab mit jeweils 500 mg (375 mg/m2) im Abstand von je einer Woche und im Anschluss 2×50 mg/d Azathioprin. In sämtlichen ambulanten Kontrollen war über 8 Wochen ein konstanter Anstieg der Thrombozytenzahlen bis auf zuletzt 70 Gpt/l (Normbereich 150 – 400 Gpt/l) zu verzeichnen. Auch die Raynaud-Symptomatik war rückläufig.

Folgerung: Eine Autoimmunthrombozytopenie ist eine potentiell lebensbedrohliche hämatologische Manifestation, die im Rahmen eines Sharp-Syndroms auftreten kann. Wenn eine hochdosierte Prednisolontherapie keinen Erfolg bringt, kann die Splenektomie eine effektive Behandlungsmethode sein. Eine sichere und suffiziente Therapiealternative bietet die B-Zell-Depletion mit Rituximab.

Summary

History and clinical findings: An 18-year-old women with mixed connective tissue disease (Sharp’s syndrome), diagnosed two years earlier, was admitted because of severe thrombocytopenia. At that time the only symptom typical for collagen disease was Raynaud`s syndrome. The patient was in good general condition, the clinical examination revealed no signs of bleeding or of splenomegaly.

Investigations: Imaging procedures showed no abnormality. The platelets were decreased to 5 Gpt/l (normal range 150 – 400 Gpt/l). The bone marrow biopsy showed a secondary immunological thrombocytopenia with an increased number of megakaryocytes.

Treatment and clinical course: Treatment with prednisolone, 100 mg/day, had no significant effect. As the patient refused splenectomy, treatment with rituximab 500 mg (375 mg/m2) per week was given over a period of four weeks, followed by azathioprine 2×50 mg/d. All tests demonstrated continuing increase of the platelet count up to 70 Gpt/l (normal range 150 – 400 Gpt/l). The signs of Raynaud`s syndrome also regressed.

Conclusion: Immunologic thrombozytopenia is a potentially life-threatening hematological manifestation of mixed connective tissue disease. If high-dosage prednisolone brings no response, a splenectomy is an efficacious treatement. B-cell depletion with rituximab offers another safe and adequate option.
