Zur antientzündlichen Wirksamkeit von Arzneimitteln aus der Teufelskralle

Zusammenfassung

Zubereitungen aus der Teufelskrallenwurzel besitzen ein den synthetischen Entzündungshemmern vergleichbares Wirkungsspektrum. Der Wirkstoffgehalt in den Arzneimitteln variiert beträchtlich. Die empfohlene Tagesdosis enthält bis zu 100 mg der wirksamkeitsmitbestimmenden Leitsubstanz Harpagosid. Das mit Wasser extrahierte Doloteffin^® und das Wurzelpulver Harpadol^® enthalten in der Tagesdosis doppelt so viel Harpagosid wie Präparate, die mit 60%igem Ethanol extrahiert wurden. Nur Teufelskrallenwurzel-Präparate, deren Wirksamkeit bei Rücken- und/oder Arthroseschmerzen nachgewiesen wurde, sind eine attraktive Alternative zu NSAR bei chronischen Schmerzen. Die Evidenzlage favorisiert Extrakte mit mindestens 50 mg Harpagosid in der Tagesdosis zur Behandlung rheumatischer Beschwerden. Je höher die Harpagosid-Dosis, umso wirksamer ist der Extrakt. Die Behandlung mit Teufelskrallenwurzel-Extrakt besitzt nach heutigem Kenntnisstand ein geringeres Nebenwirkungsrisiko als NSAR und kann bei Patienten mit Rücken- oder Gelenkbeschwerden zur Schmerzlinderung optional eingesetzt werden.

Summary

The anti-inflammatory efficacy of devil’s claw – a review Preparations from devil’s claw act similar to synthetic anti-inflammatory drugs. The content of co-active ingredients varies considerably in products from devil’s claw. The recommended daily dosage contains up to 100 mg of the co-active and leading substance harpagoside. The aqueous extract Doloteffin^® and the powder Harpadol^® prepared from freeze-dried root contain double the amount of harpagoside in the daily dosage than preparations extracted with 60% ethanol. Only devil’s claw preparations with proven effectiveness against low back and/or osteoarthritic pain are an attractive alternative option to synthetic non-steroidal anti-inflammatory drugs and can be of benefit in the treatment of chronic pain. From evidence-based view, extracts with at least 50 mg harpagoside in the daily dosage should be recommended for the treatment of pain. The higher the extract dose the more pronounced is the resulting effectiveness. Treatment with devil’s claw extract is associated with a lower risk of adverse events than treatment with traditional NSAIDs and is, therefore, a pain relief option for patients with complaints in the lower back and/or joints.
