Multiresistente Erreger – Probleme in Klinik und Praxis – Kollateralschaden einer inadäquaten Antibiotikatherapie

Infektionen mit multiresistenten Erregern werden zunehmend häufiger, sind sämtlich schwer behandelbar und mit vielen Komplikationen verbunden. Auch asymptomatische Kolonisierungen, die oft langwierig verlaufen, treten immer öfter auf. Besonders wichtig für den Praxisalltag sind in diesem Zusammenhang methicillinresistente Staphylococcus-aureus-Stämme (MRSA), vancomycinresistente Enterokokken (VRE), betalaktamasebildende Enterobakterien oder multiresistente Stämme von Pseudomonas aeroginosa. Dieser Beitrag fasst den aktuellen Stand zur Epidemiologie, Klinik und Therapie der genannten Erreger zusammen und geht darüber hinaus auf die Präventions- und Prophylaxemaßnahmen in Klinik und Praxis ein – innerhalb und außerhalb der stationären Versorgung.

The rate of infections and colonization with multiresistant bacteria is rapidly increasing worldwide. Methicillin-resistant Staphylococcus aureus (MRSA), Vancomycin-resistant Enterococcae (VRE), ESBL-positive Enterobacteriae, Pseudomonas aerugionosa among others cause severe infections in immunocompetent and especially in immunocompromised hospitalized patients. In addition, patients with asymptomatic colonization are detected with increasing frequency. The epidemiology, clinical manifestations, antimicrobial therapy and prophylactic and preventive measures are outlined in this overview. The challenges in the evaluation of preventive measures in outpatients are addressed.
