Zusammenfassung

Seit den ersten Versuchen 2001 wurden in Deutschland inzwischen zahlreiche von neurologischen Schlaganfallzentren initiierte telemedizinische Netzwerke etabliert. Sie sollen in Krankenhäusern, die über keine eigene neurologische Abteilung verfügen, die Versorgung akuter Schlaganfälle durch Einsatz teleneurologischer Konsile verbessern. Damit wurden auch systemische Lysebehandlungen in Krankenhäusern möglich, die aufgrund der geringen Zahl derartiger Maßnahmen über keine ausreichende Expertise verfügen. Seit 2011 sind neurologische Telekonsildienste auch im deutschen DRG-System erlösrelevant abgebildet. Gegenüber der Versorgung in etablierten Stroke Units ist die teleneurologische Versorgung von Schlaganfallpatienten jedoch stets nur die zweitbeste Lösung und sollte daher auf Regionen beschränkt werden, in denen ein tatsächlicher Bedarf besteht. Mit zunehmender Mobilität der verwendeten Geräte ist zu erwarten, dass Telekonsultationen auch Eingang in den klinischen Alltag in neurologischen Kliniken finden werden.

Abstract

Since the first steps in 2001, numerous German stroke centres have established telemedicine networks. Using neurological examinations via videoconference the goal is to improve acute stroke management including safe systemic thrombolysis in remote hospitals without inherent neurological departments and only marginal experience in applying this therapy due to a low number of stroke patients. Even now, tele-stroke networks have been included into the German DRG system and have become subject to medical funding. Compared to patient care in skilled stroke units, however, treating acute stroke patients via neurological teleconsultations is only the second best way and should be restricted to hospitals far distant from established stroke units. In future, caused by improved mobility of videoconference devices, teleconsultations may be increasingly used in daily routine.
