Das kolorektale Karzinom zeigt über die letzten Jahrzehnte eine deutliche Zunahme der Inzidenz und ist mittlerweile die häufigste Krebserkrankung in Deutschland. Die Prognose des kolorektalen Karzinoms hat sich hingegen in den letzten Jahren deutlich verbessert. Gründe dafür sind die Diagnosestellung in früheren Tumorstadien, die leitlinienkonforme Behandlung mit multimodalen neoadjuvanten und adjuvanten Therapiekonzepten und vor allem die signifikante Verbesserung der chirurgischen Qualität beim Rektumkarzinom durch die Etablierung der totalen mesorektalen Exzision (TME) als Standardoperation. Beim Rektumkarzinom ist die Qualität des Chirurgen ein unabhängiger prognostischer Faktor für das Auftreten eines Lokalrezidivs und für das Überleben. Bei ausgewählten Patienten in einem sehr frühen Tumorstadium kann ein lokales Vorgehen onkologisch ausreichend sein. Auch im metastasierten Tumorstadium (UICC IV) kann durch ein individualisiertes interdisziplinäres Vorgehen bei einigen Patienten eine Heilung oder ein Langzeitüberleben erreicht werden.

In Germany colorectal cancer now is the most frequent malignant disease with even increasing numbers of affected patients. Outcome and survival, however, have improved over the last decades due to more frequent diagnoses in early tumour-stages, multidisciplinary approach with neoadjuvant and adjuvant concepts according established guidelines, and the introduction of total mesorectal excision (TME) as the standard procedure for rectal cancer. Quality of surgery is the most important factor influencing tumour recurrence and survival. In selected patients local excision of early rectal tumours is possible. Even in metastatic disease (UICC IV) complete cure and longterm survival can be achieved by multidisciplinary approach in some patients.
