Zusammenfassung

Fragestellung: Auswertung von Langzeitveränderungen der Unterkieferasymmetrie bei Patienten mit hemimandibulärer Elongation (HE), welche gemäß einem zweizeitigen kieferorthopädisch-kieferchirurgischen Protokoll behandelt wurden.

Studienaufbau: beschreibende klinische Untersuchung. Umgebungsparameter und Patientenkollektiv: Patienten einer Privatpraxis. 7 von 47 Patienten, darunter 6 Frauen, die wegen der Diagnose einer schweren Unterkieferasymmetrie behandelt wurden, konnten gemäß der folgenden Kriterien in die Studie eingeschlossen werden: Diagnose einer hemimandibulären Elongation basierend auf klinischen, radiologischen und skelettszintigrafischen Untersuchungen mit qualitativ guten Orthopantomogrammen (OPG) von 6 Untersuchungszeitpunkten. Ein zweizeitiges chirurgisches Behandlungskonzept mit hoher Kondylektomie (HC) und orthognather Chirurgie (OC) wurde angewandt. Messungen der maximaler Mundöffnung wurden durchgeführt.

Methode: Die Höhe des Ramus mandibulae und die Länge des Corpus mandibulae wurden auf OPGs zu 6 verschiedenen Zeitpunkten gemessen. Die Verhältnisse zwischen der betroffenen und der nicht betroffenen Kieferseite wurden berechnet.

Resultate: Bei allen Patienten zeigte sich eine Progression der Kieferasymmetrie vor der ersten Operation, wobei dieser Befund zusammen mit dem Szintigrafie-Resultat zur Indikationsstellung für die hohe Kondylektomie führte. Danach nahmen Ramus- und Korpus-Asymmetrie im Vergleich mit den Ausgangswerten ab. Nach durchgeführter orthognather Chirurgie konnte eine stabile Symmetrie der Unterkieferkorpora sowie eine Zentrierung der skelettalen und Zahnbogenmittellinien erreicht werden. Die maximale Mundöffnung war nach der hohen Kondylektomie um 50% reduziert, nach der orthognathen Chirurgie ging die Einschränkung um 20% zurück und erholte sich praktisch komplett bis zur letzten Nachkontrolle.

Schlussfolgerungen: Eine hohe Kondylektomie zur Beendigung überschießenden Wachstums und anschließend orthognathe Chirurgie zur Korrektur der Gesichtsasymmetrie können als wirksame und zweckmäßige Therapie der HE betrachtet werden. Die maximale Mundöffnung kann sich nach chirurgischem Trauma gut erholen. Kieferorthopäden sollten eine Unterkieferasymmetrie als anormal und chirurgisch behandlungsbedürftig ansehen, wenn die Asymmetrie Progredienz zeigt und das Verhältnis zwischen betroffener und nicht betroffener Seite mehr als 10% Unterschied erreicht.

Abstract

Objective: To study long-term changes in the mandibular asymmetry of patients with hemimandibular elongation (HE) treated with 2-phase surgical-orthodontic approach.

Design: Descriptive clinical study.

Setting and Sample Population: Private practice setting. 7 (6 females) out of 47 diagnosed and treated patients with severe mandibular asymmetry were included with the following criteria: diagnosis of HE based on clinical, radiological and SPECT bonescintigraphic studies, good quality orthopantomograms (OPG) available at 6 time points, 2-phase surgical treatment (high condylectomy, HC, and orthognathic surgery, OS) and measurements of maximal mouth opening (MMO) available.

Method: Mandibular ramus height and corpus length were measured on OPGs at 6 time points. Ratios between the affected/non-affected sides were calculated.

Results: All patients had a progression of mandibular asymmetry before any operation, which finding together with the scintigraphy resulted in the decision to perform HC. Thereafter ramus and corpus asymmetry decreased compared to the initial measurements. After OS stable symmetry of corpuses and alignment of skeletal and dental midlines were obtained. MMO reduced on average 50% after HC, regained 20% after OS, and recovered almost totally at the last follow-up.

Conclusions: HC to stop excessive growth and OS to correct facial asymmetry are considered to be successful and necessary treatment for patients with HE. MMO can recover well after surgical trauma. Orthodontists should consider that mandibular asymmetry is abnormal and needs surgical treatment if the asymmetry is progressive and the ratio between affected/non-affected sides approaches 10%.
