Zusammenfassung
Die Behandlung erwachsener Patienten unterscheidet sich in einigen wesentlichen Punkten
von jener adoleszenter Patienten. Fehlendes Kieferwachstum, Kiefergelenksprobleme,
Systemerkrankungen mit entsprechender Dauermedikation, Zahnsubstanzverlust, ausgedehnte
Restaurationen, Zahnimplantate und reduziertes Parodont mit oder ohne aktive Parodontitis
sind einige dieser Faktoren, die spezielle Berücksichtigung in der Therapieplanung
verlangen. Sie beeinflussen sowohl die prätherapeutische als auch die kieferorthopädische
Phase. Vor allem die Berücksichtigung der parodontalen Verhältnisse ist eine Grundvoraussetzung
für eine erfolgreiche Therapie. Eine aktive Parodontitis muss vor Therapiebeginn behandelt
sein und reizlose Verhältnisse während der gesamten aktiven Behandlung aufrechterhalten
werden.
Ein reduzierter Zahnhalteapparat verändert die biomechanischen Verhältnisse des Zahn-Parodont-Alveolarknochen-Systems
und erfordert eine adäquate Reduktion der wirkenden Kräfte. Unter Beachtung dieser
Faktoren ist auch bei parodontal kompromittierten Patienten eine kieferorthopädische
Behandlung ohne zusätzlichen Attachmentverlust möglich, wie der vorliegende Fallbericht
zeigt.
Abstract
The treatment of adult patients differs in some important aspects from those of adolescent
patients. Lack of jaw growth, temporomandibular joint problems, systemic diseases,
long-term medication, tooth loss, extensive restorations, dental implants and reduced
periodontal attachment with or without active periodontitis are some of the factors
that require special attention when planning orthodontic treatment. They influence
the pretreatment as well as the orthodontic treatment phase. Special consideration
of the periodontal conditions is a prerequisite for successful therapy. Any active
periodontitis must be under control before and inflammation-free conditions must be
maintained throughout active orthodontic treatment. Reduced periodontal attachment
changes the biomechanical conditions of the tooth-periodontal-alveolar bone-system
and requires an adequate reduction of the applied forces. Considering these factors,
also in periodontally compromised patients orthodontic treatment without additional
attachment loss is possible, as the present case report shows.
