Zusammenfassung

Die Therapie einer Klasse-III-Dysgnathie stellt nach wie vor eine große Herausforderung für den Kieferorthopäden dar. Dabei kann die Behandlung einer skelettalen Klasse III entweder rein kieferorthopädisch oder kombiniert kieferorthopädisch-kieferchirurgisch erfolgen. Die Wahl der Therapieform hängt von der Ausprägung der skelettalen Diskrepanz, vom Wachstumsmuster, vom Alter des Patienten und nicht zuletzt von dessen ästhetischen Wünschen ab. In diesem Artikel wird die Therapie einer jugendlichen Patientin mit einer sowohl skelettal als auch dental ausgeprägten Klasse III dargestellt. Da eine kieferchirurgische Intervention für die Patientin und deren Eltern nicht infrage kam, wurde die Behandlung mittels modifizierter Osteosyntheseplatten und intermaxillärer Gummizüge zusätzlich zu einer festsitzenden kieferorthopädischen Apparatur durchgeführt. Die Ergebnisse zeigen die Korrektur des umgekehrten Frontzahnüberbisses und der dentalen Klasse III im Eckzahn- und Molarenbereich. Zusätzlich konnten die skelettalen Anzeichen der Dysgnathie abgeschwächt werden. Die Gesichtsästhetik erfuhr durch die Verringerung der positiven Lippentreppe eine eindeutige Verbesserung.

Abstract

The Treatment of Class III malocclusions is a very challenging task for the orthodontist. Skeletal discrepancies can be corrected with either orthopedics, orthodontic camouflage or orthognathic surgery. The decision for a certain treatment modality is not only dependent on the skeletal pattern. Patients̛ age, chief complaint and expectations should be discussed and considered. This case report presents a young girl with a severe Class III syndrome. Orthognatic surgery was not an option neither for the patient nor her parents. Therefore she was treated by using intermaxillary elastics to orthodontic miniplates and a fixed orthodontic appliance. The treatment outcome shows the correction of the anterior crossbite and the class III molar relationship. Skeletal aspects of the class III malocclusion were decreased. Facial aesthetics were improved through a reduction of the facial concavity.
