Bindungstheorie in der Palliativmedizin – Auswirkungen standardisierter Bindungsdiagnostik mit dem Adult Attachment Picture System auf die psychische Befindlichkeit der Patienten

Zusammenfassung

Das Erleben einer schweren unheilbaren Krankheit in einer palliativen Situation ist eine potenziell bindungsrelevante Situation. Bisherige Veröffentlichungen, die sich mit den Implikationen der Bindungstheorie für die Situation am Lebensende beschäftigen, stützten sich auf Selbsteinschätzungsverfahren bzw. Beobachtung, um Aussagen über die verinnerlichten Modelle von Bindungen der Patienten zu treffen. In dieser Studie wurde daher untersucht, ob das Adult Attachment Picture System als standardisiertes Fremdeinschätzungsverfahren zur Erfassung von Bindungsrepräsentanzen im Kontext der Palliativmedizin einsetzbar ist. In einer Prä-Post-Messung wurden die Auswirkungen der Diagnostik auf die Befindlichkeit untersucht. Die Ergebnisse zeigen, dass 28 % der Patienten im Untersuchungszeitraum auf einer Palliativstation mit dem Instrument untersucht werden konnten. Hinsichtlich demografischer Variablen unterschieden sich die untersuchten Patienten nicht von den übrigen auf Station. Die mit dem mehrdimensionalen Befindlichkeitsfragebogen erfasste Befindlichkeit lieferte keine Anhaltspunkte für das Vorliegen einer psychischen Belastung durch die Diagnostik. Eine standardisierte Bindungsdiagnostik mit einem Fremdeinschätzungsverfahren ist nach ersten Erkenntnissen grundsätzlich im Kontext der Palliativsituation möglich und nicht belastend.

Abstract

Attachment Theory posits a predisposition for attachment behavior not only in early childhood but also in case of severe illness. Adult attachment representations of palliative medicine inpatients have been described based on observation or questionnaire-studies. However, research concerning standardized measures is rare. The present study evaluates application of a standardized instrument, the Adult Attachment Projective, in palliative medicine. Pre-post-data provide information about changes of the inpatients mental state during the diagnostic procedure. 28 % of the palliative medicine inpatients in the sample period participated in the study. The pre-post-measurement revealed no significant changes of participants´ mood during the diagnostic procedure. Our results indicate that standardized measurement of adult attachment is in principle applicable in palliative medicine.
