Zusammenfassung

Die chronisch obstruktive Lungenkrankheit (COPD) stellt eine der häufigsten Komorbiditäten bei Patienten mit kardiovaskulären Erkrankungen dar, was nur teilweise auf den gemeinsamen Risikofaktor Zigarettenrauchen zurückzuführen ist. Für die Betreuung des Patienten im klinischen Alltag ist eine möglichst genaue Abgrenzung der COPD vom Asthma bronchiale notwendig, da sich die Therapie dieser beiden Erkrankungen in wesentlichen Punkten unterscheidet. Dazu gehört der Einsatz von Betablockern und inhalativen Steroiden. Zwar handelt es sich sowohl bei Asthma als auch bei der COPD um entzündliche Erkrankungen von Lunge und Atemwegen. Der Einsatz inhalativer Steroiden ist jedoch nur beim Asthma Grundlage der Therapie, kann bei der COPD sogar schädlich sein. Daher muss die Beseitigung der inhalativen Noxen im Vordergrund der Behandlung der COPD stehen. Da das gemeinsame Auftreten von COPD und arteriosklerotischen Erkrankungen die Prognose der Patienten ungünstig beeinflusst, sollte die systematische Suche und Therapie einer COPD bei arteriosklerotischen Erkrankungen (und umgekehrt) implementiert werden. Die medikamentöse Therapie der COPD zielt auf die symptomatische Behandlung der Luftnot, die Verringerung der Exazerbationen und die Beeinflussung von Komplikationen.

Abstract

The chronic obstructive pulmonary disease (COPD) represents one of the most relevant comorbidities in patients with cardiovascular disorders. The close association can only in part be explained by the shared risk factors smoking, older age and male gender. The comorbidity impairs the prognosis of both cardiovascular diseases and COPD. Therefore, systematic screening for either disorder is warranted. For clinical and therapeutical purposes the clear discrimination of asthma and COPD is of crucial importance. Treatment with inhaled steroids (ICS) and beta blockers differs substantially between asthma and COPD. Although the pathophysiology of both disorders is characterized by airway inflammation, ICS are the basis of the controller therapy only in asthma. In contrast, ICS may be deleterious in COPD patients. Therefore, the elimination of inhalative noxes — especially smoking — is the mainstay of COPD therapy. The pharmaceutical therapy of the COPD focusses on the relief of symptoms, such as dyspnea, expectoration or physical limitation, the reduction of exacerbations and complications.
