Die positive T-Welle in V1 als Hinweis auf eine signifikante KHK

Upright T-wave in V1 as predictor of significant coronary artery disease

Zusammenfassung

Anamnese und klinischer Befund: Der 56-jährige Patient wurde aufgrund von Sodbrennen seit dem Vormittag, zu dem im Tagesverlauf ein thorakales Engegefühl und Schweißausbrüche hinzukamen, am frühen Abend in der Notaufnahme vorstellig. Kardiovaskuläre Risikofaktoren waren ein arterieller Hypertonus und Adipositas. Die klinische Untersuchung ergab keine auffälligen Befunde.

Untersuchungen: Laborchemisch zeigten sich bis auf eine leichte Kreatininerhöhung auf 1,19 mg/dl keine Auffälligkeiten. Das in der Aufnahmesituation abgenommene Troponin I ultra sensitiv war negativ (< 0,006 ng/ml), CK und CK-MB normwertig. Im Aufnahme-EKG imponierte ein Sinusrhythmus mit einer Herzfrequenz von 68/min, eine nicht signifikante ST-Hebung in V2, eine T-Erhöhung in V1 und V2. Die T-Welle in V1 war positiv (> 0,15 mV) und größer als die T-Welle in V6.

Therapie und Verlauf: Der Patient wurde auf unsere Intermediate-Care-Station aufgenommen. Eine Troponin-Kontrolle ca. 4 Stunden nach Krankenhausaufnahme ergab ein positives Troponin I ultra sensitiv von 1,519 ng/ml (Norm: 0,02–0,06 ng/ml). Bei Nicht-ST-Hebungsinfarkt (NSTEMI) erfolgte am Folgetag eine Koronarangiographie. Diese ergab eine koronare 3-Gefäß-Erkrankung mit 95 %iger Stenose des Ramus interventricularis anterior, welche; das Gefäß wurde mit einem medikamentenfreisetzenden Stent versorgt.

Folgerung: Eine positive T-Welle in V1 tritt beim gesunden Mensch eher selten auf und kann ein Hinweis auf eine koronare Herzerkrankung sein. Eine Größendifferenz zwischen der T-Welle in V1 und V6 zugunsten der T-Welle in V1 ist ein Hinweis auf eine KHK mit Beteiligung des RIVA. Bei entsprechender Symptomatik und positiver T-Welle in V1 sollte an eine kardiale Ischämie gedacht und ggf. frühzeitig eine weitergehende Diagnostik und Therapie eingeleitet werden.

Abstract

History and clinical findings: The 56-year-old man came to our emergency department because of heartburn, chest tightness and sweating. Cardiovascular risk factors were obesity and hypertension. Clinical examination revealed no significant abnormalities.

Examinations: Laboratory results showed no pathologies except a slightly elevated creatinine (1,19 ng/dl). Troponin I ultra sensitive was negative (< 0,006 ng/ml), CK and CK-MB not elevated. The 12-channel-ECG revealed sinus rhythm with a heart rate of 68/min, a non-significant ST-elevation in V2 as well as elevated T-waves in V1 and V2. The T-wave in V1 was positive (> 0,15 mV) and bigger than the T-wave in V6.

Treatment and course: The patient was monitored at our intermediate-care ward. The troponin level was checked again after about four hours and showed a troponin of 1,519 ng/ml (norm: 0,02 – 0,06 ng/ml). NSTEMI was diagnosed and coronary angiography was performed next morning, demonstrating coronary three vessel disease with 95 % occlusion of the left anterior descending (LAD).

Conclusion: An upright T-wave in V1 can be an indicator for coronary artery disease (CAD). A bigger T-wave in V1 than in V6 can indicate CAD  with LAD  involvement. Therefore, in patients with thoracic pain and a positive T-wave in V1 cardiac ischemia should be considered.
