Zusammenfassung

Andauernde und beeinträchtigende Erschöpfung, die durch eine bekannte medizinische Erkrankung nicht erklärt werden kann, stellt das Kernmerkmal des chronischen Erschöpfungssyndroms dar. Die in Forschung und klinischer Praxis etablierten Klassifikationsansätze unterscheiden sich vor allem hinsichtlich geforderter Zusatzsymptomatik und zeitlichen Verlaufsvorgaben, implizit aber auch hinsichtlich zugrundeliegender ätiologischer Konzeptualisierungen. Für die Nähe zu anderen funktionellen somatischen Syndromen spricht die hohe Komorbidität mit anderen, organisch nicht hinreichend erklärten Körperbeschwerden. Entsprechend sollte sich die Diagnostik nicht auf eine sorgfältige somatische Abklärung der Erschöpfungssymptomatik begrenzen, sondern auch das Vorliegen weiterer körperlicher Beschwerden, psychosoziale Faktoren (spez. subjektive Krankheitskonzepte) und Auswirkungen des Gesamtbeschwerdebilds umfassen. Empfehlungen zum diagnostischen Vorgehen und zur Therapie geben die kürzlich von medizinischen und psychologischen Fachgesellschaften (AWMF, DGPs) in Deutschland entwickelten Leitlinien. Psychotherapie (v. a. kognitive Verhaltenstherapie) und therapeutisch angeleitete gestufte Aktivierung stellen evidenzbasierte Therapieoptionen dar.

Abstract

Enduring and disabling fatigue that cannot be explained by a known disease is the main characteristic of chronic fatigue syndrome. Several definitions do exist, and classification approaches vary regarding supplementary symptoms, time course, and by implicit concepts of aetiology. CFS can be considered as a functional somatic syndrome, e. g. supported by the high rates of comorbid bodily complaints and syndromes that lack clear medical explanation. Accordingly the diagnostic process should not be limited to the thorough physical examination, but also address additional somatic complaints, psychosocial factors (specifically subjective illness beliefs), and impairments. Recently German medical and psychological societies provided treatment guidelines for functional somatic syndromes. Cognitive behavioural therapy and graded activity are evidence based treatment methods for CFS.
